What are dermal fillers?

Hyaluronic acid fillers restore volume, define structure and soften folds. Hyaluronic acid is a sugar molecule your skin already produces; the gels are cross-linked versions of it, formulated at different thicknesses for different jobs — soft and pliable for lips, firmer and more supportive for cheekbones and jawline.

We use Juvéderm and Restylane. Both are hyaluronic acid, and both are reversible.

Who places your filler, and why it matters more here than with Botox

You choose your injector when you book, and booking a physician means a physician places your filler.

Filler carries a risk that neurotoxin does not. If gel enters an artery rather than the tissue beside it, it can block the blood supply — causing skin necrosis, or in rare cases permanent vision loss. Serious vascular complications occur in roughly 0.001% to 0.05% of injections, so this is genuinely uncommon. But the American Academy of Ophthalmology has catalogued 198 published cases of filler-related vision loss, 83% of them from hyaluronic acid. Rare is not the same as never.

What decides the outcome is what happens in the next few minutes. Hyaluronidase — the enzyme that dissolves hyaluronic acid — is the antidote, and time is the variable: reversing an ophthalmic artery occlusion is understood to depend on treatment within roughly 90 minutes. International consensus guidance is that every clinic injecting hyaluronic acid must keep hyaluronidase available for immediate use and be able to reconstitute it on the spot.

We do keep it on site. And a physician is in the building who can recognize an occlusion and treat it, rather than sending you somewhere else while the clock runs.

We inject under ultrasound

We use a high-frequency handheld ultrasound (Clarius) in the office and we scan before and during filler treatment. This is not standard practice in aesthetics, and it changes four things.

It shows your anatomy rather than average anatomy. Every textbook diagram of the facial arteries describes a typical course. Real vessels vary, and the danger zones are exactly where that variation matters. Doppler imaging locates the vessel and its depth in the area we are about to treat, which replaces an assumption with information.

It shows filler that is already there. Hyaluronic acid can persist far longer than most patients expect, sometimes years, and people are often unsure what they had, where, or when — particularly if it was placed somewhere else. Ultrasound shows what is actually in the tissue, which changes what should be added and where.

It confirms blood flow afterward. Doppler lets us check that circulation in the treated area is intact rather than waiting to see whether a problem develops.

It means less hyaluronidase when filler needs dissolving. Hyaluronidase does not distinguish between filler and the hyaluronic acid your own tissue makes, so using more than necessary has a cost. Published dosing work indicates that targeting the deposit under imaging allows substantially lower doses than dissolving blind.

Ultrasound does not make filler risk-free and we would not claim it does. It replaces guesswork with a look, which is a meaningful difference in a treatment where the serious complications come from putting something in the wrong place.

Why we don't offer liquid rhinoplasty

Non-surgical nose reshaping is popular, and we decline to do it.

The nose sits at the top of the published risk list for vascular complications, alongside the glabella and forehead. The vessels are small, the tissue is tight so there is little room to absorb injection pressure, and the arterial supply connects to the circulation that feeds the eye. The reported cases of blindness after filler cluster in exactly this territory, while the lower face — lips, chin, jawline — accounts for very few.

Plenty of clinics offer it. We think the risk-to-benefit is wrong for a cosmetic change that surgery achieves permanently and safely, and we would rather tell you that than take the booking.

What can filler treat?

• Lip enhancement, hydration and definition
• Cheek and midface volume
• Jawline contouring and chin projection
• Under-eye hollowing (tear trough)
• Nasolabial folds and marionette lines

Most treatments take one to four syringes depending on the area and how much change you are after. We would rather build gradually across visits than place too much in one sitting.

Rewards programs — and making sure your points actually land

Which manufacturer rewards program applies depends on the product going in, not on the practice.

Allē is Allergan's program and covers Juvéderm. ASPIRE is Galderma's and covers Restylane. Since we use both products, it is worth asking which applies before you book — if either suits your treatment equally well, the answer may be worth factoring in.

We report your treatment to the manufacturer so the points post to your account rather than quietly going uncredited. If you already have an Allē or ASPIRE account, mention it when you book and we will attach it.

Cherry financing is also accepted if you would rather spread the cost across a course of treatment.

What should I expect during and after treatment?

Thirty to sixty minutes. We apply topical numbing beforehand, and the fillers themselves contain lidocaine, so most patients describe pressure rather than pain.

You will see a result immediately, but it is not the final result — swelling accounts for some of what you see in the first week, and the filler settles into place over about two weeks. Bruising is possible and usually resolves in one to three days. Skip strenuous exercise and alcohol for 24 hours.

Do not book filler within two to three weeks of a wedding, a photo shoot or anything you cannot reschedule. This is the single most common planning mistake patients make. If you have a date in the calendar, our wedding and event consultation plans the whole timeline backward from it.

Can filler be dissolved?

Yes, and that matters. Hyaluronidase breaks down hyaluronic acid within a day or two, so if you dislike the result it is correctable rather than something you live with. We locate the deposit under ultrasound before dissolving, which lets us target it rather than treating the whole area blind. Permanent and semi-permanent fillers offer no equivalent, which is why we do not use them.

When filler isn't the right answer under the eye

We are cautious about under-eye filler. It suits some patients very well and makes others look heavier, and which group you fall into is an anatomy question rather than a preference. Where the tear trough is genuinely hollow and the skin holds product, filler works. Where the skin is thin or crepey rather than empty, PRF is usually the better tool, and we will say so. And where the problem is skin excess or a true lower-eyelid fat pad, neither one fixes it — that needs an oculoplastic evaluation, usually at Wills Eye Institute or the oculoplastic service at Penn, and we refer rather than camouflaging a surgical problem with product.
