PhillyMedClub Concierge Primary Care gives you direct access to Dr. Ghavami and Dr. Raneiro for routine medical needs, without the waits that have become normal in primary care.

The premise is simple. Most primary care practices carry two to three thousand patients per physician. At that volume, a same-week appointment is not realistic and a returned phone call the same day is a favor rather than a standard. We limit our panel to 100 patients, which is what makes the rest of this page possible.

What membership includes

  • A comprehensive annual exam — unhurried, and long enough to actually cover your history
  • Direct access to your physician by phone or email, not to a call center
  • Appointments within 36 hours when you need to be seen
  • A response within a few hours for questions that do not need a visit
  • Lab and imaging orders when they are clinically indicated
  • Medication refills handled by phone or email where appropriate, rather than requiring a visit

What it costs

For 2026, membership is $1,500 per person or $2,000 per couple, billed annually.

That is the membership fee. Individual visits through the year are billed separately and are generally comparable to what a copay would be — see the section below on how that interacts with your insurance, because it is usually more favorable than people expect.

What membership is not

Two things worth being straight about, because vagueness here causes friction later.

It is not unlimited care. Membership covers access, expedited scheduling and routine visits through the year. It is not a license for weekly appointments and daily phone calls, and a practice that promised that could not deliver the rest of it. If your needs are genuinely that intensive, we should talk about whether this is the right arrangement for you.

It is not insurance and does not replace it. You still need coverage for hospital care, specialists, imaging, surgery and medication. Membership buys access to us. It does not buy coverage for everything else.

How this works alongside your insurance

We do not bill insurance and we do not handle prior authorizations. But that does not mean your insurance is irrelevant here, and this is the part most concierge practices explain poorly.

You get a superbill for your visits. Submit it to your insurer yourself and those visits typically count toward your annual deductible — which matters more than most people realize if you are working toward it.

You can pay with an HSA or FSA card in the office. The visit cost is generally comparable to what a copay would be, and paying it with pre-tax dollars is usually the most efficient route available to you.

A PPO plan works best with this model. With a PPO we can refer you directly to the subspecialist who is right for the problem, without routing the referral through an insurer-assigned primary care physician you may never have met. If you have an HMO, that referral pathway is more constrained, and it is worth discussing before you join rather than after.

Where primary care meets the rest of what we do

Having your physician and the rest of the practice under one roof changes what a routine visit can lead to.

Fatigue, low libido, poor sleep and stalled body composition are the sort of thing men raise at an annual exam and then never follow up on, because the next step means another practice and another wait. Here it does not. Where testosterone turns out to be the issue, testosterone replacement therapy is managed by the same physicians who ordered the bloodwork. The same is true of medical weight loss, and of the hormonal, thyroid and nutritional contributors that turn up when someone actually looks.

None of that is bundled into membership — they are separate services. What membership buys is a physician who already knows your history when the conversation starts.

Who this suits

People who want a physician who knows them, answers them, and can see them when something is actually wrong.

It suits patients managing ongoing conditions who are tired of explaining their history to someone new each visit. It suits people whose work makes a three-week wait genuinely costly. And it suits anyone who has left a seven-minute appointment realizing they never got to the thing they came in for.

It is a poor fit if you rarely see a doctor and are content with an urgent care clinic when you do — you would be paying for access you do not use.
